MAIL:
Use the form provided to you by your Sponsor. Complete the form and include it with your check, money order, or credit card information, to cover the cost of product. Mail to the address shown on the form.
FAX:
Complete the fax form, write your check just as if you were going to mail it. Attach your check to the form (you can tape it to the form). Make a copy of the complete form and check, and fax the copy to: (205) 661-1955. Be sure to include a copy of your registration form as provided to you by your Sponsor.
